The Health Insurance Portability and Accountability Act of 1996 (HIPAA) requires CONCERN to notify potentially affected individuals of improper disclosures of Protected Health Information (PHI).  On June 6, 2023 our email system was hacked and they gained access to some client’s personal information. We discovered the hack and excluded them on June 15, 2023.    We were unable to notify approximately 14 people about this due to not having any address or contact information or because letters were returned.  The information accessed by the hacker was only the first and last name of the client.  There is no indication that the information was used for any illegal purpose.  Nevertheless, in accordance with Pennsylvania and Federal law, we are notifying you.  This message constitutes substitute notice for those individuals we attempted to notify but were unable to reach.  Only individuals receiving Adoption services were involved.  If you are receiving other services from CONCERN you were not impacted.   We sincerely regret that this incident occurred and are taking appropriate measures to prevent it occurring in the future, including our ongoing efforts to educate staff.  If you have any questions or believe that you may have been one of the individuals with an unknown address, please contact us at this toll free number 1-855-234-3168 to inquire if your information was accessed.
